For the Apollo missions, why couldn't they just launch the Command/Service module already docked with the LM?
For the Apollo missions going to the Moon, as well as upcoming Orion missions also headed for the Moon and relying on a very similar design -
The Command/Service module had to separate from the last Saturn V stage containing the LM, then flip around, dock with the Lunar Module and extract it from that last Saturn V stage. Why couldn't they just launch them docked?
This was because of aerodynamcs and also life support reasons.
At launch, the crew were in the most forward portion of the spacecraft, the command/service portion. This gave them access to the escape system and if anything went wrong they could jettison all the other equipment below them.
The Command / Service module they rode in was tapered almost to a point so as to be aerodynamic. This was not true of the lunar lander, which was designed only to fly in the vacuum of space and to land on the moon.
That is why they had the lander stowed beneath the command / service module.

Also, they were going to have to dock the 2 together again after the lander seperated to land on the moon and return.

So, they might as well do it twice. If they had trouble doing it the first time (before anyone was actually in the lander) then they could have aborted the mission more safely.
